Feedback form for players?

Hi all,
I have a question for you all, how do you handle the process of getting feeback from your players? I am about 30 sessions into a Shadowdark campaign with my in-person group and we are having a great time and I don’t see us stopping anytime soon, at least that is the impression I get from the more vocal players. Now I am wondering if everyone feels that way and also generally, what aspects of the game the players enjoy particularly, which parts not so much etc. I would like to do some sort of anonymous simple query, so everyone can speak their mind (it is a large open table group, not everyone is present every single session, etc) and I can find areas in my GMing to work on. Is anyone aware of some existing tools towards this end or should I make my own?

I usually just use a Google Forms poll to get the feedback I’m looking for. It works great, you can make it anonymous if you want.

I just use a typical Google Form as well. I try to focus on things like accessibility, environment, feeling comfortable with topics and the people, etc.

Maybe this is callous but I don’t focus too much on the gameplay. I will ask if players are having moments they enjoy and if they have feedback, but back in the day I asked more in-depth “is this amount of combat ok” sort of questions which imho is completely useless.

I think Stars and Wishes is a great start. Stars are moments they specially liked, and Wishes are things they want to see in the future.

I think this way avoids asking about low points or things that need polishing to keep it positive, but I suppose you could ask for worst moments too.

When I use it, it’s almost always near the end of a campaign and I’m surveying them to decide what to play next. So it’s often stuff about what genre they’re interested in, and logistics of how we want to play. Often that might include some questions about what they enjoyed about our previous campaign, or what things they want to see more of.
